Clinical Nutrition and Management

Evaluate the postprandial metabolic response, and cope with a disease!

Searching for management methods and food compositions that can contribute to the prevention / treatment of disease

Postprandial hyperglycemia results in initiation of oxidative stress, and induces islet hypertrophy with hyperinsulinemia in pre-diabetic subjects. Thus, not only decreasing postprandial hyperglycemia, but also conserving insulin secretion, may be effective in the prevention of type 2 diabetes mellitus and metabolic syndrome. We focus on isomaltulose (Palatinose®), known as the isoform of sucrose and a slowly digested disaccharide. For future studies, we plan to examine the detailed effects of isomaltulose in vivo, especially in insulin-related tissues.

Furthermore, malnutrition is a common clinical feature in hemodialysis patients, including decreased dietary protein and energy intake and/or increasing energy expenditure (EE) and catabolism. For future studies, we plan to assess patients’ EE and respiratory quotients during dialysis.

1. Research on the isomaltulose that reduces visceral fat and protects pancreatic islets function, and analyzes the mechanism of action
We expect that not only postprandial lower early insulin secretion, but also prolonged modest insulin secretion by isomaltulose supplement, might be important in protecting against beta-cell dysfunction and adipocyte hypertrophy. Our results suggest that isomaltulose might be useful in the treatment of type 2 diabetes mellitus.
2. Research on the evaluation of the metabolic status using functional foods
We plan to focus on the delta index of plasma glucose and insulin levels at 30 minutes after loading of functional foods; we expect that we may be able to evaluate a compensatory metabolic change in healthy individuals.
3. Research on the evaluation of metabolic changes for nutrients during dialysis in hemodialysis patient
Chronic hemodialysis patients have significantly lower respiratory quotients at the end point of dialysis procedures than at same time on a non-dialysis day. Thus, hemodialysis patients might be enhanced a catabolic action during the dialysis.
